How long does a panic attack last? And why it always ends
The short answer: a panic attack peaks within about 10 minutes and most ease within 20–30 minutes. However bad it feels, it is temporary — and it will end on its own. That single fact is one of the most calming things to hold onto mid-attack.

Why the intense part is so short
A panic attack is a surge of adrenaline — the fight-or-flight response firing without real danger. Your body simply cannot keep adrenaline that high for long. Like a wave, it rises, crests, and falls. The peak is brief by design; your job isn't to stop it, but to stay standing while it passes.
Why it can feel much longer
Two things stretch the experience:
- The comedown. After the peak you can feel drained, shaky, tearful or “off” for anywhere from a few minutes to a couple of hours. That's the adrenaline clearing — not a continuing attack.
- Fighting it. Watching the clock and demanding “stop now!” adds fear on top of fear, which can trigger fresh waves. Paradoxically, letting the wave be there is what lets it fall.
What if it seems to last for hours?
A single, continuous attack doesn't last hours. If it feels that way, it's usually one of these:
- Rolling waves — several attacks close together, with brief dips between.
- Anticipatory anxiety — the tense, on-edge state of fearing the next attack, which can last a long time but isn't the peak itself.
Naming which one is happening helps: the acute wave is short; the background anxiety is what to work on over time (and it responds very well to CBT).
What makes an attack shorter
- Name it: “This is panic. It's adrenaline, not danger. It will pass.”
- Slow the exhale: in for 4, out for 6 — a longer exhale nudges the body toward calm.
- Ground your senses (5-4-3-2-1): pull attention out of your head and into the room.
- Don't run if you're safe — riding it out teaches your brain the place is safe.
When it might not be panic. If chest pain radiates to the arm or jaw, you faint, symptoms worsen with exertion, or it's the first time ever — don't wait it out as panic; seek emergency care (911 in the US, or your local number).
